Types of pensions for doctors

Medical workers receive a labor pension, like most citizens of the Russian Federation. At the same time, work experience plays a more important role here.

Not every employee of a medical organization has the right to early retirement. To do this, you need to hold a certain medical position.

Long service pension for medical workers

How to calculate a preferential pension

In addition, the salary is equal to the established coefficient.

To calculate pension payments for long service, you need to familiarize yourself with the following example:

  1. For example, a doctor worked for 20 years in a row in a hospital. He worked in the surgical department as a doctor, so his length of service is equal to the product of 20 and 1.5, that is, 30. Consequently, the citizen can begin to receive pension payments for his length of service.
  2. When calculating labor pension provision, a fixed payment (4823 rubles) and a pension point indicator (78) are taken into account.
  3. In total, the citizen has accumulated 70 points. Consequently, his long-service pension will be equal to the product of 70 and 78, summed with 4823, that is, 10,283 rubles.

Depending on the subject of the Russian Federation in which the person was employed, he may be assigned a preferential additional payment corresponding to the regional coefficient.

Registration of early pension

For medical workers, the algorithm for assigning pensions is indistinguishable from the procedure for registering pensions for everyone else. A citizen is required to consistently perform the following actions:

Reform in Russia - latest news for health workers

The 2019 pension reform also applies to health workers. For them, the new bill number 350 of October 3, 2018 introduces a deferment period for the appointment of preferential pensions for long service.

The duration of this period will increase every year. The final standards will be set in 2023; they will be equal to 60 months .

This means that those doctors who accumulate the required preferential length of service in 2023 and later will retire only after 60 months, that is, in 2028.

The standard for the introduced deferment will increase according to a certain algorithm. From 2019, this indicator will increase by 12 months annually, until it reaches 5 years in 2023. In 2019 and 2020, it will be possible to receive pension payments six months ahead of schedule.

Preferential pension for medical workers in 2020

The pension reform that started in Russia also affected preferential pensions for medical workers. In 2020, healthcare employees who have earned the required length of service will not immediately retire.

Changes

Since 2019, the standard deferment introduced has been in effect. According to the scheme proposed by the government, the duration of the deferment increases annually by 12 months and will amount to 60 months by 2023. Taking into account the proposed changes, doctors and junior medical staff who have earned the necessary preferential length of service by this time will take advantage of it in 5 years - in 2028.

There is an exception for employees who earned the required length of service in 2020. They are entitled to a preferential pension 6 months earlier than the deadline determined by law. Who is not affected by innovations?

Legislative innovations do not apply to specialists working in certain conditions:

  • particularly harmful conditions – among radiologists and laboratory technicians in x-ray rooms;
  • harmful working conditions for workers in psychiatric, tuberculosis, oncology and infectious diseases hospitals.

The rules for retirement and assignment of pensions to these categories of health workers in 2020 will remain unchanged. The deferment does not apply to them. The basis is Federal Law No. 350, which came into force on October 3, 2018.

Required experience

Employees of medical institutions can count on early registration of state benefits, regardless of age and total work experience. The main requirement was and remains work experience in a medical institution. It amounts to:

  • 30 years for doctors working in the city;
  • 25 years for doctors practicing in rural areas.

When calculating the amount of social security, a fixed payment and pension points are taken into account - the calculation principle is the same for all categories of citizens. In addition to the amount of the benefit, regional additional payments may be assigned. Their power coefficient in regions, territories and districts is set independently, so the amounts may differ significantly.

Registration procedure

Social payments are issued in accordance with the general procedure. The sequence of action is as follows:

The state provides other privileges to healthcare workers. Among them:

  1. Medical care at the place of work. It is paid for by the employer.
  2. Shorter working day. The work week is 24, 30, 33 or 36 hours (depending on the category of the employee).
  3. Special meals for personnel of X-ray departments, surgery and intensive care.
  4. Additional days to paid leave – from 14 to 35 days (depending on the place of employment).
  5. Additional payments and bonuses, the amount of which is established by the administration of the healthcare institution.

Doctors practicing in rural areas are provided with housing. It is paid for by the local budget. After 10 years of service at this place of work, a health worker has the right to live in this house or apartment permanently, even after retirement due to length of service or age.

To apply for a long-service pension, doctors will need to have:

  1. Medical work experience according to clause 20, part 1, art. 30 of Law No. 400-FZ of December 28, 2013 “On Insurance Pensions”, namely:
    • 25 years - in rural areas or urban settlements;
    • 30 years - mixed experience (in cities, rural areas and urban areas) or only in cities.
  2. Minimum required IPC value on the date of registration of payments in accordance with Part 3 of Art. 8 of Law No. 400-FZ. Every year the IPC standard increases by 2.4 points, therefore:
    • in 2020 you need to have 18.6 points;
    • in 2021 - already 21, etc.;
    • and from 2025, the minimum value of the IPC will be fixed - 30 pension coefficients.

When taking into account medical experience, the position and name of the institution in which the health worker works is of particular importance, since the preferential length of service includes time spent working only in the relevant positions and institutions provided for by law. Let us note that the calculation of special length of service is carried out according to a certain order, for example, for some categories of workers the accounting is “a year and a half”.

Pension reform for doctors (by length of service)

Since 2019, the pension reform has also affected medical workers - for them, the new law No. 350-FZ of October 3, 2018 provides for the introduction of a deferment period for receiving a preferential long-service pension. The new law adopted in 2018 does not provide any conditions for the type of activity of medical workers during the period for which the possibility of receiving a pension is postponed. The main condition for preferential retirement of medical workers (doctors, nurses, midwives, etc.

How to calculate preferential length of service for retirement for doctors

The rules for calculating preferential length of service for health workers differ depending on the period of such work - they are regulated by legislative acts for each corresponding period. For work activities carried out from January 1, 2002 to the present, the following procedure for calculating medical experience applies:

  • Preferential length of service is taken into account in calendar order , i.e. a year of work in a position specified in the list is taken into account as a year of preferential service. It does not matter what clinical profile is indicated in the name of the healthcare institution, and the departmental or territorial affiliation of the organization is not important.
  • In some cases, length of service is not taken into account “year by year”, but on a preferential basis:
    • A year of work in a rural area or in an urban village is counted as 1 year and 3 months (this is important for health workers with mixed experience, for whom the standard is set at 30 years of medical experience).
    • For the list of positions established by law, medical experience is counted as “a year and a half .
  • Work experience before 01.11.1999 is counted as a preferential benefit in full, and after this date only those periods of work during which the standard working time according to labor legislation (normal or reduced) were met.
  • If the work was carried out in several positions (or institutions) during a part-time working day, then it will be taken into account in the preferential length of service only if the total amount of working hours corresponding to the full rate for one of the positions is worked out.
  • In healthcare institutions of the Russian Federation abroad, length of service is counted on a general basis.

For which positions is medical experience counted as a year and a half?

The legislation provides for a list of positions and healthcare institutions, a year of work in which is counted as one and a half years. For periods of work from 01/01/2002, this rule applies to the following health workers:

  1. Surgical departments, hospitals, burns, oncology, orthopedic, otolaryngological, microsurgical departments as:
    • operating medical specialists of any profile, incl. medical directors;
    • operating rooms med. nurses (including elders), midwives (including elders), honey. nurses in purulent surgery, trauma, burn and other departments.
  2. Departments (mobile ambulance teams, wards, groups) of anesthesiology-reanimation, resuscitation and intensive care, namely:
    • anesthesiologists, resuscitators (including heads);
    • honey. ward sisters, incl. seniors;
    • honey. nurse anesthetists.
  3. Bureau court. honey. examinations, central courts. honey. examinations, court. honey. laboratories, etc. as:
    • forensic doctors, incl. managers;

Resolution No. 781 approved a complete List of healthcare institutions and titles of positions of doctors and paramedics. personnel, in which a year of employment is counted as 1 year 6 months. In order for preferential medical experience to be taken into account according to this scheme, it is necessary that the entry in the work book corresponds to that specified in this list.

Is studying at a medical school (residency, internship) included in the length of service?

The length of service includes periods during which insurance contributions are paid to the Pension Fund (Part 1, Article 11 of Law No. 400-FZ of December 28, 2013 “On Insurance Pensions”).

Therefore, the time during which a citizen received an education will be counted toward preferential length of service only if during his studies he held a full-time position and received a salary (for example, he was on leave with pay).

This is possible, for example, when taking advanced training courses, internship, residency, etc. In addition, the length of service may not be taken into account in calendar order, but, for example, a year and a half, if during training the health worker took part in operations (in some cases, such a decision is made only in court).

Accordingly, if during the training period the physician was not registered under an employment contract (accordingly, insurance premiums were not paid), such a period will not be included in the medical experience.

A deferment is a set period of time that increases the time required for beneficiaries to work.

In accordance with established legislative standards, by 2023 the deferment amount will reach the required maximum of 5 years.

  • At the same time, the increase in the deferment value will occur in stages: every year the term of early appointment will be postponed by 1 year (except for the first two years - 2019 and 2020), until the final value is set at 5 years (in 2023).

This will increase the amount of service established for today:

  • for health workers of rural organizations and institutions located in urban settlements, from 25 to 30 years;
  • for other categories of beneficiaries from 30 to 35 years.

After admission to the internship, an employment contract is concluded with the student. An employment relationship arises, and deductions will be made to the Pension Fund for the student. This means that an internship will bring you closer to retirement. If it is not considered work, then such training is included during insurance periods.

Medical workers are divided into categories. The higher this indicator, the more experienced the doctor is considered. To advance up the career ladder in the medical field, you will have to work for one period or another. It depends on the category for which the employee is applying.
