What does a birth certificate consist of?

A special form of the established form with coupons and stubs that pay for medical care - that’s what a birth certificate is; accordingly, it consists of several parts (to facilitate visual perception, they are highlighted in light green and pink):

1. Stub , which remains in the medical institution that issued the document. It is filled out in the antenatal clinic, usually at 30 weeks of pregnancy. It confirms that the certificate has been issued to the patient.

2. Coupon No. 1 - to pay for outpatient monitoring of a pregnant woman. According to it, the Social Insurance Fund transfers funds for the work of doctors, medical staff, and for the purchase of necessary equipment.

The expectant mother is given the remaining part of the certificate, which she must submit to the maternity hospital along with the exchange card and other necessary documents.

3. Coupon No. 2 - for payment for maternity hospital services. It also pays for food and care for the mother and newborn. If a woman wants to stay in a single room, she must pay the remaining amount.

This part of the certificate is filled out and submitted to the FSS. For paid births, this part is not filled out.

4. The main document confirming the provision of medical care. This part is given to the young mother without additional coupons.

This part confirms that she received full medical services during pregnancy and childbirth, and her child - up to 1 year.

5. Coupon No. 3-1 - to pay for services for dispensary observation of a newborn child during the first six months of life.

After discharge from the maternity hospital, the newborn is registered at the children's clinic. The first consultation is carried out no later than 3 months of age.

6. Coupon No. 3-2 - for the second 6 months of medical observation of the baby. It transfers money to pay for child care services during the second half of the year from the moment of birth.

On the reverse sides of the last two coupons there are additional columns in case more than one child is born (twins, triplets, and so on).

Receipt procedure

The expectant mother receives a certificate of temporary incapacity for work for the employer and accrues benefits to her - this is when a birth certificate is issued, simultaneously with sick leave. This happens at 28–30 weeks. Registration takes place at the antenatal clinic, where the woman is registered. And if it was observed in different institutions, then in the place where it was observed for a longer time.

  • The main condition for issuing a certificate at the antenatal clinic is that it must be continuously monitored for 12 weeks.
  • To receive a document, a woman presents:
  • The counterfoil and coupon No. 1 remain in consultation.
  • Coupon No. 2 and the rest of the document are presented at the maternity hospital along with the exchange card.
  • Upon discharge from the maternity hospital, coupons 3-1 and 3-2 are transferred to the children's clinic to pay for the child's observation procedure, and the mother is left with only the main part of the certificate to confirm that she received medical services during pregnancy and childbirth.

If the pregnant woman was not registered at the consultation and did not receive the document there, then it will be issued at the maternity hospital. The first coupon in it will be canceled, no one will receive money from it.

Is it possible not to receive a birth certificate?

We figured out why a birth certificate is needed, and it turns out that it is required when a woman is observed under compulsory medical insurance. So why do those who use paid healthcare need it?

Yes, often a certificate is not issued if the pregnancy is observed in a private clinic on a paid basis. However, a birth certificate allows you to reduce the cost of pregnancy and childbirth and subsequent monitoring of the child until he reaches 1 year. To receive coupons, contact your local antenatal clinic after the 30th obstetric week of pregnancy.

If a woman has not received a certificate for premature birth, then the maternity hospital, based on the exchange card, itself requests all the data from the clinic where the woman was observed.

And the mother has the right to issue a coupon at the children's clinic. Thanks to him, the services of a pediatrician who monitors the child’s condition will be paid for.

Is it possible to get it after 30 weeks?

  1. What should you do if the required 30 weeks for processing the document have passed?
  2. If a birth certificate was not received upon registration at the antenatal clinic or the woman was observed in a private clinic on a paid basis, then the maternity hospital or the district children's clinic in which the child was registered before reaching 3 months has the right to issue it.
  3. We conclude: a woman will not be left without this document, even if the deadline for it in the antenatal clinic when registering has passed.

Birth certificate amount in 2020

We are interested in the 2020 birth certificate: how much is allocated for it? Its total cost is now 12,000 rubles (it has not changed for several years, but has increased since 2020). Of them:

  • 3,000 rubles are sent to the consultation where the woman was observed before giving birth;
  • 6,000 rubles are transferred to the maternity hospital;
  • 2 times 1000 rubles are paid to the children's clinic for each six months of dispensary observation of a baby in the first year of life.
  • 1000 rubles - for the provision of legal, psychological and medical-social assistance - to pay for the labor of lawyers, medical psychologists (psychologists) and social work specialists.

These amounts are paid from the federal budget. It is impossible to receive money using individual coupons or in cash in total, since the funds are directed specifically to pay for the services of health workers.

Certificates are issued only by municipal medical institutions and serve to pay for their services provided to the population free of charge. If you were observed for pregnancy or gave birth in a private clinic, then you cannot reimburse expenses for paid services using a certificate.

The absence of such a document for various reasons does not give state medical organizations the right to demand that a woman pay for medical services at her own expense.

How to receive payment under a birth certificate

The program is financed from the budget of the Social Insurance Fund, and funds to pay for the services of medical institutions are transferred directly to their account.

But there are exceptions. Certificate coupons are not subject to payment in several cases:

  • if it was issued in violation of the requirements;
  • if it is issued to a woman who does not have identification documents;
  • if it is issued by a medical institution that provides services on a paid basis;
  • in case of death of mother or child.

IMPORTANT! The woman cannot cash the birth certificate if she gets money for it. This document is not a type of financial assistance to young mothers.

Birth certificate and exchange card - differences

This stimulates the work of these institutions, and gives every woman in labor a guarantee that both the antenatal clinic and the maternity hospital will be interested in providing qualified services specifically to her.

IMPORTANT! The amount of money under this document from the Social Insurance Fund is transferred only to the medical institution on the basis of the corresponding coupon presented from it. Withdrawal of these funds by other methods or with the participation of third parties is IMPOSSIBLE.

How to get a birth certificate

Where does the receiving process begin (when can you get it, who issues it, in what forms, necessary documentation, features of receiving it in a paid clinic).

In order to be able to freely receive a “red slip”, the expectant mother needs to register with the antenatal clinic. This refers to a budgetary institution. The basis for issuance is observation of a pregnant woman for at least 12 weeks. This means that for this purpose, it would be more expedient to register no later than 25-26 weeks.

It is important that a woman undergo regular examinations, take the necessary tests and undergo procedures. Otherwise, problems may arise with the issuance of the certificate. The responsibility for issuing a birth certificate rests with the doctor managing her pregnancy.

What should a pregnant woman do if she wants to be seen in a paid clinic? The algorithm of actions may be different:

  • Find a clinic that has the right to issue this document. To obtain this information, just call there by phone.
  • If the clinic does not have such a right, then at the last date you can come to the state antenatal clinic and, upon discharge from the private clinic, receive the required completed form.
  • The certificate is not necessarily issued at the clinic. It can be obtained at the maternity hospital, where Coupon No. 1 will already be listed as used

Why is this sheet not issued in full in the maternity hospital? This only happens if the pregnant woman was observed in a paid clinic and personally paid for all the necessary procedures and services, or if she was registered for less than 12 weeks, or even not registered at all. In such cases, pregnancy monitoring services cannot be paid for by the state.

What documents are needed to obtain

To obtain a birth certificate, you must provide copies of the following documents to the medical institution:

  • passport (any other identification document, up to 14 years of age - birth certificate);
  • compulsory health insurance policy (CHI);
  • insurance certificate of compulsory pension insurance (SNILS).

Every Russian woman can apply for a birth certificate, regardless of her status, social status, work experience or age.

Citizens of other countries have the right to be issued a “red sheet” and to receive full medical services based on it. To do this, they must have a temporary residence permit or residence permit.

Stages of filling out and receiving coupons, their purpose, where and when to go with them

The full algorithm of actions with the pink form can be seen in the standard case, when it is issued in a women's clinic after 12 weeks of monitoring a pregnant woman:

  1. The spine filled out in the LCD before the pregnant woman is transferred to the maternity ward. As a rule, from the 30th week, but possibly earlier if issuance is necessary. This receipt confirms that the patient has received MS and remains at the place of issue.
  2. Coupon No. 1 is filled out at the same medical institution, immediately detached, and then sent to the Social Insurance Fund office to receive payment.   4. After discharge from the maternity hospital, the newborn baby is registered in the children's department of the clinic. Here you will need Coupon No. 3-1 , which will be used to pay for the services of a medical institution for monitoring the child in the first 6 months, and Coupon No. 3-2 - for monitoring up to a year. On the reverse sides of these coupons there are extra columns in case of the birth of several children (twins, triplets, etc.).
  5. The BIRTH CERTIFICATE remains with the mother as confirmation that she received full medical services during pregnancy and childbirth, and her child (children) - for up to 1 year.
  • For a more clear understanding of the purpose of each part of the RS form, a table has been compiled:
  • The total maximum amount of money transferred to medical institutions under a birth certificate is 11,000 rubles.

A medical institution may not receive compensation from the state for a certificate in case of improper execution of the document, or in case of violations in the provision of medical services. In the event of the death of a mother or child in the maternity hospital, the maternity hospital will also not be paid the amount under Coupon No. 2.

Obtaining a certificate in a maternity hospital or children's clinic

Situations cannot be excluded when problems arise with the timely receipt of a birth certificate, such as:

  • Observation of a pregnant woman in a private clinic that does not have the right to issue this certificate.
  • Observation in a state housing complex for less than 12 full weeks or serious violations of visiting a doctor (frequent absences, failure to take tests, etc.).
  • Premature birth.
  • Adoption of an infant (up to 1 year).
  • Loss of a previously issued certificate.

Depending on the situation (except for adoption), the maternity hospital either makes a request to the housing complex in order to restore or transfer the certificate to it, or issues a new one without filling out Coupon No. 1.

In the case of adoption of a baby under one year old , as well as in the case when a woman gives birth in a paid clinic that does not have government support, the certificate is issued in the children's clinic where the baby will be observed. Coupon No. 1 and No. 2 will be marked “cancelled.”

In these cases, the list of documents provided for issuing a birth certificate does not change.

What is a pregnant woman's exchange card and why should it not be confused with a birth certificate?

Another mandatory document for a pregnant woman is an exchange card. A pregnant woman's exchange card (WEC) is created from the moment a woman registers for pregnancy in the housing complex and is filled out until she is discharged from the maternity hospital.

Unlike the RS, which has a financial function, the OKB has an informational nature. It reflects complete, reliable information about the patient’s pregnancy, with all the nuances.

That is why it is so important; on its basis, doctors predict what the birth will be like, what should be taken into account so that it goes well.

A pregnant woman’s exchange card is the most important document of a woman in labor, her kind of medical “passport”.

In the maternity hospital, the OKB must also be carefully filled out, indicating all the features of the course of labor and the postpartum hospital period. Based on these data, the gynecologist to whom the woman will go after being discharged from the maternity hospital (as a rule, this is the same doctor who cared for the pregnancy) will be able to understand the overall picture and prescribe treatment for the woman in labor’s recovery period.

Also in the maternity hospital, the card is filled out by a pediatrician. Data about the newborn is entered, which is then transferred to the children's clinic.

Are funds issued under a birth certificate to the owner herself?

The birth certificate is aimed at stimulating the activities of medical staff to provide full, qualified care for the mother and her child. Therefore, funds from it go towards compensation for used medicines and wages for health workers. All this is necessary to improve the quality of medical care.

Birth certificate funds do not constitute financial assistance to mothers.

A patient who independently pays for services provided for by the MS regulations will not be able to claim compensation for her expenses. Also, keeping a mother who has given birth in a separate room in the maternity ward is an additional service not included in the MS list. It will not be paid by either party.

What does a birth certificate provide: what services and rights do mothers receive?

This document is valid in all state, regional and municipal medical institutions (antenatal clinics, clinics, maternity hospitals). To use it 100%, you need to know the entire list of MS services provided by the state.

A woman has the right to independently choose a medical institution, as well as choose doctors for supervision.

What does a birth certificate provide?

Maternity hospital and other institutions

Deductions from the specified source are earnings for doctors and the entire institution. The money allows us to reward staff and build the material and technical base of the organization.

It is clear that all state medical institutions, in any case, are financed without any certificates - according to budget standards. And all women in labor, if necessary, will be admitted to these consultations and maternity hospitals without certificates.

However, it is worth remembering that the certificate is beneficial to both parties - both the doctors and the woman in labor. For the former, payments from the certificate are a significant additional source of income. Is a birth certificate required?

No, not required. If you are observed and give birth in a private paid clinic (or in a public clinic, but also for money), then the matter will probably not come to the point of registration at all.

If you still give birth in a state maternity hospital and without using paid services, and you did not receive a certificate, then the institution will accept you without a certificate and then issue it yourself.

If you still plan to use the services of a maternity hospital where the certificate is accepted, then it is better to apply for it in advance. This is guaranteed to allow you to exercise the right to choose the maternity hospital of your choice.   • – tests: urine, blood, feces, smears, HIV, RW, glucose tolerance test;
  • – free vitamins for pregnant women;
  • – other.
  • In addition, the certificate includes examination for congenital and genetic diseases of the fetus and continuation of pregnancy in a hospital (other assistance).
  1. Maternity hospital: ward, food, childbirth, observation, medications, vaccinations, etc.

Implementation procedure

The tear-off parts that the document includes should be left in medical institutions. Through the social insurance fund, these coupons will be exchanged for money.

Doctors fill out the coupons and the certificate itself.

At week 12, the counterfoil is filled out, and then coupon number 1 - both of these actions are carried out in the antenatal clinic (where the certificate is stored during this period).

Coupon number 2 and the certificate itself are filled out in the maternity hospital. The coupon remains in the institution, and the certificate is in the hands of the mother. The birth certificate contains the date, place, time of birth, gender, weight and height of the child (other). The remaining coupons are filled out at the children's clinic.

The certificate includes 5 parts:

  1. A counterfoil with a name - the woman’s identification data is filled in: address, medical policy number, SNILS and date of issue. After use, this part remains in the healthcare facility.

In 2020, the amount of funds under the certificate that can be used to pay for the services of medical workers is 12 thousand rubles :

The amount under the certificate cannot be reduced or increased and does not depend on the number of newborns, since the document is issued in the name of the mother. If more than one child was born, then all of them will be entered in the appropriate fields in Coupon No. 3.

The procedure for issuing and processing a birth certificate

Birth certificates for women are issued by antenatal clinics, maternity hospitals and children's clinics.

Attention! Paid institutions do not issue a certificate, since this program is aimed at improving the professionalism of doctors and the quality of work in public hospitals.
