The result of your meeting with him is recommendations on how to quickly and efficiently adapt to new “life conditions” and at the same time remain calm and happy by the end of the school year.

Surgeon

We go to him so that he can examine the child’s posture and the development of the entire musculoskeletal system as a whole. After all, studying at school will place a large burden on the spine. Is there a curvature of the spine, is there flat feet, or maybe you need physical therapy or massage?

Dentist

Probably the most problematic doctor in terms of children’s willingness to see him. Usually, parents of their timid offspring prepare for a dental examination well in advance. But, alas, it’s impossible without him.

After all, preschool age is precisely the time when baby teeth fall out and are replaced by permanent ones. And here you have an incorrect bite, and molars lost in the gums, and many all sorts of “surprises” that can interfere, distracting you from the educational process.

I am ready to stumble upon the indignation of those parents who prefer not to get vaccinated in principle. They have such a right to refuse vaccines. But, unfortunately, in Russia, schools, with rare exceptions, respect this right to choose whether to vaccinate a child or not, and in most cases, to put it mildly, they insist on observing the rules for vaccination established in Russian medicine.

What can they give as a load?

In the process of undergoing a medical examination before entering school, “the secret becomes apparent.” Problems often emerge that parents had no idea about, because the child did not complain. So do not resist if you are additionally sent, for example, to a dermatologist, speech therapist, endocrinologist, gastroenterologist or neuropsychiatrist.

You can, of course, give up on such examinations. Your child will be admitted to school without the conclusions of additional specialists, but is it worth turning a blind eye to problems with children's health?!

What is PMPC?

Some schools have such a special “authority” called PMPK. This is a test for the first-grader and for his parents. In fact, this is not a “formidable beast” at all, but a simple psychological, medical and psychological commission that the school assembles before enrolling those who want to come to 1st grade.

It usually includes a school teacher, psychologist, speech therapist, psychiatrist and defectologist, and their common task is to determine the most appropriate way of teaching and identify the need for additional classes.

Children and their parents come to the commission, they are asked various questions about life, family, the world around them, and the children perform simple tests.

As a result, speech, thinking, motor development, coordination and intelligence level are assessed.

Let me note again that this is not available everywhere, and the PMPK’s verdict is not a verdict on parents, but only a recommendation. But you must admit, participation in such a commission is one of the opportunities for us, parents, to “spread the straw” in time and not put an unbearable burden on the child.

At first glance, it seems that worries have increased again.

Absolutely not! Well, just a little, if your baby does not go to kindergarten, then you will have to visit a medical institution and undergo a medical examination of the future first-grader yourself.

After all, even though medical certificate 026/u is not paranoia at all, and the educational institution has no right to demand it from you, but this is by law, but in practice it is completely different.

Every year, before children enter kindergarten or school, parents ask themselves: “Do I need to undergo a medical examination?” and “Where can I do this?” Even medical professionals sometimes disagree on these issues. Often, in clinics and childcare centers, parents are provided with inaccurate information.

We talked about this with the director of the multidisciplinary clinic "Medicate", candidate of medical sciences Atyasheva Lyudmila Nikolaevna , who specially prepared for parents the latest legally valid information on the issues of medical examinations of children before kindergarten and school.

Before a child enters any general educational institution (kindergarten, school, Suvorov or cadet school), it is necessary to undergo a preliminary medical examination. Lately there have been a lot of rumors that these types of examinations have been cancelled, simplified, and are no longer required. Is it so?

“Indeed, there is currently a lot of talk and debate in the media and at the level of federal authorities about not specifically highlighting medical examinations before school and kindergarten, since the law on medical examination of children provides for annual examinations in state children's clinics. No one doubts the quality of these examinations in our clinics, so for now these are just conversations and working discussions. No regulatory documents have been adopted in this regard, and there are no plans to do so in the near future.

Moreover, in August 2017, a new Order of the Ministry of Health of the Russian Federation No. 514n to replace the previously existing one, which regulates medical examinations in age groups before kindergarten and school. According to this Order, some changes in the scope of necessary examinations and inspections are provided, and the timing of the implementation is clearly regulated. This order came into force on January 1, 2018.

— This Order states that to complete this medical examination, “examinations and results of examinations by medical specialists may be used, included in the medical documentation of a minor (child development card), which are not older than 3 months from the date of the examination by a medical specialist and (or ) examinations, for children over 2 years of age.” In the case of obtaining a medical card for a child under 2 years of age, such periods are narrowed to 1 month. So how does the new order differ from the previous one?

— The order strictly regulates the set of necessary examinations by medical specialists and studies for each age group. It does not have significant differences from the previously valid one in terms of the need for an inspection, the timing and place of its conduct. However, there are still some differences!

Before entering kindergarten, the child must undergo an examination by 7 specialists (stage 1): neurologist, ophthalmologist, ENT doctor, pediatric surgeon, pediatric urologist-andrologist (boys), obstetrician-gynecologist (girls).

Recently, relaxation has been allowed for examinations of girls before kindergarten by gynecologists, which seems reasonable. Although there is no clear recommendation in the Order excluding this type of inspection. An examination by a dentist is recommended.

Of the necessary studies, only general blood and urine tests remained.

  • Now for kindergarten there is no requirement to take tests for glucose, blood or feces for helminths! You also don't need any
  • Ultrasound examinations.     

Before a minor enters school, it is necessary to undergo an examination by 9 specialists (stage 1): neurologist, psychiatrist, ophthalmologist, ENT doctor, pediatric surgeon, pediatric urologist-andrologist (boys), obstetrician-gynecologist (girls), orthopedic traumatologist (indications). after examination by a pediatric surgeon). An examination by a dentist is recommended.

Future schoolchildren are no longer required to undergo examinations by a dermatologist and pediatric endocrinologist.

  1. Necessary examinations: general blood and urine tests, ultrasound of the abdominal organs and kidneys, ECG (electrocardiography) and EchoCG (ultrasound of the heart).
  2. According to the new order, it is no longer necessary to conduct an ultrasound of the thyroid gland and ultrasound of the reproductive organs in boys and girls.
  3. Before entering a kindergarten or school, a medical examination of a minor is completed by an examination by a pediatrician, who summarizes the identified abnormalities or diseases in the child, gives an opinion on the child’s state of health, necessary recommendations for treatment, or refers the child to additional examinations and consultations with specialists (stage 2 of the examination).

— Parents often come to our clinic with non-standard lists of necessary examinations and examinations offered for completion at the clinic at their place of residence. Such demands are illegal.

The required list of stage 1 medical examination of a minor before school and kindergarten, according to the current Order, does not include, for example, examinations by a traumatologist-orthopedist before kindergarten, a pediatric endocrinologist, a pediatric cardiologist, a dermatologist, X-ray examinations, and others not mentioned by me above.

But at stage 2 (after consultation with a pediatrician) “if a minor is suspected of having a disease (condition), the diagnosis of which could not be established at stage 1 of the medical examination,” the child may be recommended any additional examinations and consultations with specialists not included in the main list medical examination.

Do I need to undergo examinations by a speech therapist and psychologist?

— Examinations by a speech therapist and a child psychologist are not mandatory when a child undergoes a medical examination before kindergarten and school. These types of inspections are not included in the list of required services.

However, some educational institutions ask to undergo examinations by a child psychologist and speech therapist for a medical-pedagogical commission (regulated by local regulations and not included in the medical examination).

Today, parents prefer not to waste time and energy in queues at public clinics. Is it necessary to undergo a medical examination at a municipal clinic, or can I go to paid medical centers for this?

— This Order clearly establishes the right of parents to choose: to undergo these types of medical examinations in clinics at their place of residence or in private medical centers licensed for these types of medical activities.

If medical documentation is correctly filled out based on the results of a medical examination, educational institutions do not have the right to refuse parents to accept documents from a private medical center (clinic).

The timing of these medical examinations of minors is clearly regulated: stage 1 - no more than 20 working days, stage 2 - no more than 45 working days.

When carrying out stages 1 and 2 of examination in state medical institutions, all examinations (consultations) of specialists and examinations necessary for the child are provided free of charge as part of the program of state guarantees of free provision of medical care to citizens.

Are there any special examination conditions for guys who are preparing to enter the cadet or Suvorov military school?

— Parents of children preparing to enter the cadet or Suvorov military school contact us with a request for a medical examination. Order of the Ministry of Health of the Russian Federation No. 514n also regulates the ability of parents to conduct these examinations in medical institutions of any form of ownership.

However, this document cannot regulate the necessary list of examinations and examinations for a child’s admission to a given educational institution, since this list is determined by the local regulations of the educational institution and may differ significantly between schools.

Before going for a medical examination, I recommend that you familiarize yourself with the requirements of a particular educational institution for conducting a medical examination before admission.
